MASSACHUSETTS MARITIME ATHLETICS NCAA COMPLIANCE FORMS
Massachusetts Maritime and the NCAA require all prospective student-athletes to complete all of the forms listed below before participating in a varsity athletics program at the Academy. Prior to your arrival on campus for the upcoming Fall 2023 semester, please download the forms listed below, complete all required sections and mail them to:

Please note that all prospective student-athletes WILL NOT BE ALLOWED to participate in any official practices or team activities until the above paperwork is completed.
A synopsis of the compliance forms follows:
- NCAA Student-Athlete Statement – please read carefully and thoroughly. Print your name and write “Massachusetts Maritime” in the space for institution. Sign and date all of the listed items. ON PART IV – sign only Question 2A, which indicates you have not had a positive drug test. ONLY SIGN Question 2B under Part IV if YOU HAVE HAD A POSITIVE DRUG TEST.
- NCAA Drug-Testing Consent – please read carefully and thoroughly. Print “Massachusetts Maritime” in the space for institution. Sign and date all of the listed items and read the entire list of NCAA banned substances for the 2012-13 academic year. IF YOU HAVE NOT REACHED YOUR 18TH BIRTHDAY, please have your parent or guardian sign below your name.
- HIPPA/Buckley Amendment Consent Form – please read carefully and thoroughly. Print your name and “Massachusetts Maritime” as your institution at the top. Print your name, signature and date at the bottom.
- Massachusetts Maritime Sports Information Student-Athlete Questionnaire – please read carefully and thoroughly. Please complete all sections on the front and back and sign and date at the bottom of the second page. Your MMA e-mail address will be your firstname.lastname@maritime.edu. If you do not know the name of your daily or hometown newspapers, please leave the questions blank.
- Massachusetts Maritime Athletics Social Media Policy – please read carefully and thoroughly. If you have accounts on various social media platforms (Twitter, Facebook, You Tube, etc.) and you perform an action or activity on them that causes harm to your teammates, your program, the Department and the Academy, there are consequences. This policy is also listed on our website and is part of your Student-Athlete Handbook. Please sign and date the bottom of the form.
